A report from a high-powered commission today blasted the government, regulators, and Tokyo Electric Power Company (TEPCO) for not anticipating and preventing the crisis at Fukushima Daiichi Nuclear Power Plant triggered by a powerful earthquake and tsunami in March 2011.
